For your convenience the forms may be printed from this page and completed at home. Bringing your completed forms to your appointment will expedite registration. Alternatively, come 10 minutes early to fill out registration forms. Patient Information Form (pdf) If you have had x-rays, an MRI, CT scan, EMG or other diagnostic studies ordered by your primary care doctor or referring doctor, these studies and the corresponding report document will need to be brought by you to the appointment. With very few exceptions there is no mechanism for other facilities to deliver these studies to your specialist at Mt. Tam. They are your personal and private health information and will need to be carried by you when you come for your first consultation. |
