Your Visit

First Visit

The physicians and surgeons at Mt. Tam Orthopedics believe in maximizing the time used to examine you and answer your questions regarding your condition and treatment options.  As such, for your first visit, we ask that you fill out the form to the right titled "Initial Visit".  In addition, please fill out the relevant form for the body part which is being evaluated.  i.e. "Shoulder", "Hip", "Knee",  "Ankle", or "Other".  Please bring both of these forms with you when you come for your visit.

If your visit is related to a work injury, please also fill out the form titled "Work Related Injury" in addition to the other two forms required.

Patient Forms

The following forms can be downloaded to your computer and printed at home. By filling out these forms ahead of time, you will save a significant amount of time during your visit. Please be prepared to provide this information to our office along with your current medical insurance information. If you have any questions when filling out these forms, please do the best you can and our staff will assist you with your questions on the day of your visit.
