Your Visit

First Visit

The physicians and surgeons at Mt. Tam Orthopedics believe in maximizing the time used to examine you and answer your questions regarding your condition and treatment options. As such, for your first visit, we ask that you fill out the forms to the right titled “Initial Visit”, “Medical History” and “HIPAA Privacy Notice.” Please bring these forms with you when you come for your visit.

If you are returning for follow-up on a condition you have already been treated for, please complete the form titled "Follow Up Medical Form" no more than 1 day before your appointment. Please bring this form with you when you come for your visit.

If your visit is related to a work injury, please also fill out the form titled “Work Related Injury” in addition to the other forms required.

Patient Forms

The following forms can be downloaded to your computer and printed at home. By filling out these forms ahead of time, you will save a significant amount of time during your visit. Please be prepared to provide this information to our office along with your current medical insurance information. If you have any questions when filling out these forms, please do the best you can and our staff will assist you with your questions on the day of your visit.
